WHAT ARE BIOSTIMULATORS?

Traditional dermal fillers (like Juvederm or Restylane) work by adding volume directly with hyaluronic acid gel. They’re immediate but temporary, lasting 6–18 months before your body dissolves them.

Biostimulators work differently. Instead of adding a foreign gel, they contain microscopic particles that trigger your skin’s own fibroblasts — the cells responsible for producing collagen and elastin. Over the weeks and months following treatment, your skin rebuilds its own internal structure, creating results that are:

  • Gradual and natural-looking (no sudden overnight change)
  • Longer-lasting — typically 18 months to 2+ years

SCULPTRA® AT SIMPLY INJECTABLES RI

What it is

Sculptra is made of poly-L-lactic acid (PLLA) — a biocompatible, biodegradable polymer used safely in medicine for decades. FDA-approved for facial volume restoration, it works by stimulating your skin’s own fibroblasts to build new collagen. The product dissolves completely over time, leaving only your body’s own collagen behind. There is no permanent foreign substance — just your skin, rebuilt from within.

How it works — Richard’s four-treatment framework

Most practices tell patients they need “one vial per decade of life.” I think that’s an oversimplification that leaves out a lot of important factors. Here’s the framework I actually use with my patients — based on what I observe clinically after each treatment:

Treatment 1 — The Glow: Collagen production is triggered. Patients notice improved skin quality, hydration, and a subtle radiance. Volume change is minimal at this stage.

Treatment 2 — The Scaffold: New collagen fibers begin forming a structural network beneath the skin. You’ll feel firmer. The skin starts to lift. This is where patients often say “something is different but I can’t put my finger on it.”

Treatment 3 — Early Revolumization: Meaningful volume begins to return, particularly in the cheeks, temples, and jawline. Friends and family start noticing. The changes look natural because they are — it’s your own collagen.

Treatment 4 — Full Revolumization: This is where the most dramatic, structural transformation occurs. Patients who reach this stage often report that this is the best their skin has looked in years. The results are real, reproducible, and yours.

Why I don’t rely on the “one vial per decade” rule: A 40-year-old who has never smoked, wears SPF daily, and has healthy genetics may need far fewer vials than a 40-year-old with years of sun exposure. Collagen loss is driven by age, yes — but also by UV damage, smoking, stress, diet, and genetics. We’re not treating a number. We’re treating a person.

I also tell patients that Sculptra response is individual. In my clinical experience: about 80% are consistent responders and produce collagen predictably. About 10% are underproducers — they need more product to see the same result. And about 10% are overproducers — they respond dramatically and need careful calibration to avoid over-correction. RADIESSE® AT SIMPLY INJECTABLES RI

What it is

Radiesse is made of calcium hydroxylapatite (CaHA) microspheres suspended in a gel carrier — a substance that occurs naturally in the body (it’s the same mineral found in bones and teeth). It is one of the original biostimulators. I’ve been using Radiesse for nearly 15 years, and it has recently come back into widespread favor thanks to a technique called hyperdilution.

Hyperdiluted Radiesse — old product, new technique

When Radiesse is used at full concentration, it provides immediate volumizing lift. That has its place. But the technique producing the most exciting results right now is hyperdilute Radiesse: we dilute the product with saline or lidocaine so it spreads across a larger surface area. The result is less “filler effect” and more “skin quality upgrade” — the calcium microspheres still trigger fibroblast activity and collagen production, but across a broader treatment zone with a more natural result.

Where I use Radiesse — and where I prefer Sculptra

My strong clinical preference is to use Radiesse in the neck and décolletage, and I rarely use it in the face. That’s a personal provider preference. For facial biostimulation, I prefer Sculptra’s gradual, controllable collagen response. For the neck and chest — areas that are notoriously difficult to treat and often undertreated — hyperdiluted Radiesse is, in my opinion, the best non-surgical option available. Nothing else I’ve seen comes close.

I want to be honest about the neck, because I think patients deserve a straight answer: the best treatment for a significantly lax neck is surgery. A skilled plastic surgeon can accomplish what no injectable can. I tell patients that directly. But for those who want meaningful, real-world improvement without surgery — or want to delay it — hyperdiluted Radiesse is exceptional.

I have had patients come to me after spending thousands on radiofrequency devices, ultrasound treatments, and CO₂ lasers — convinced nothing would work. After a Radiesse series in the neck, many have told me they couldn’t believe they spent money on those other treatments first. For the neck, my recommendation is simple: a biostimulator (preferably Radiesse) or surgery. Everything else in between is, in my clinical opinion, unlikely to give you results that justify the investment.

A NOTE ON PRF & PRP TREATMENTS

You may have come across Platelet-Rich Fibrin (PRF) and Platelet-Rich Plasma (PRP) in your research into biostimulatory treatments. These are autologous treatments — meaning they use a small sample of your own blood, which is then processed and reinjected to deliver growth factors and stimulate collagen production.

At Simply Injectables, we explored PRF/PRP as part of our treatment offering and followed its results carefully over time. After extensive clinical observation, we made the decision to discontinue these services.

We no longer offer PRF or PRP injections at Simply Injectables. While we respect the science behind these treatments and understand their appeal, we observed too much inconsistency in outcomes — patients were investing significant money and time without reliable, predictable results. Our commitment has always been to treatments that deliver consistent, high-quality outcomes for every patient. PRF and PRP simply did not meet that bar for us in the injectable category.

We made the deliberate choice to focus exclusively on Sculptra® and Radiesse® as our biostimulatory injectables — both of which are FDA-approved, extensively studied, and deliver the reliable, reproducible results our patients deserve. When you invest in a treatment at Simply Injectables, you should be able to expect a predictable outcome. That’s the standard we hold ourselves to.

A WORD ON ARIESSENCE PDGF+ AND INJECTION USE

Ariessence Pure PDGF+ is a product we are proud to use at Simply Injectables — specifically for our microneedling treatments, where it delivers outstanding results for skin quality, hydration, and radiance.

You may have seen or heard about Ariessence PDGF+ being used as an injectable biostimulator at some clinics. We want to be transparent with you about where we stand on this.

Ariessence Pure PDGF+ is currently labeled for topical use only — the product label explicitly states it is NOT indicated for injection. It does not currently have FDA approval or established clinical indications for use as an injectable biostimulator. While we understand the scientific rationale for why some injectors are exploring this use, and we recognize that the growth factor technology is genuinely promising for the future, we cannot in good conscience offer it as an injection at this time. We believe in taking the moral high ground — even when others in the industry may not.

At Simply Injectables, patient safety is never negotiable. We will not use products in ways that fall outside their labeled indications, regardless of anecdotal results or industry trends. If and when Ariessence PDGF+ receives appropriate clinical validation and indication for injectable use, we will revisit this with the rigor and transparency our patients expect.

Are biostimulators safe?

Yes. Both Sculptra and Radiesse have extensive safety records and are FDA-approved. Sculptra’s PLLA material has been used in medical procedures for over 30 years. Radiesse’s CaHA is a naturally occurring substance in the body. At Simply Injectables, all treatments are administered by a board-certified Physician Assistant with over 13 years of experience.

How long do biostimulators last?

Sculptra results typically last 2 years or more. Radiesse generally lasts 12–18 months. Individual results vary based on metabolism, lifestyle, and the number of treatment sessions.

Do biostimulators hurt?

Discomfort is minimal. A topical numbing cream is applied before treatment, and both products contain or are mixed with lidocaine for additional comfort. Most patients rate their experience as very tolerable.

Can I combine biostimulators with Botox or fillers?

Absolutely — and many of our patients do. A common protocol combines Botox for dynamic wrinkles, hyaluronic acid fillers for immediate targeted volume, and Sculptra for overall facial collagen restoration. Richard will design a combination plan tailored to your goals.
